Bluetooth is wireless technology. It has nothing to do with sound, except that on phones, it it used to send music files between devices. The iPod touch doesn't have built in speakers, so no music or videos or YouTube without headphones; unless you just want to watch a video without listening... But I do quite often surf the web without headphones...
So, essentially, YES. You need headphones to hear anything (except for the clicks when your typing, etc.)
